Currently all donations to the department must be sent to the Baylor Oral health Foundation. You can make payments through one of the following channels:
CHECK: Mail a check to BOHF at 3600 Gaston Avenue, Suite 1151, Dallas TX 75246. Please write the check to BOHF and put 'Orthodontic Support Fund' in the memo line of the check. Send donations to the attention of either Carla Siegemund or Linda Piper.
CREDIT CARD: Call either Linda or Carla at: 214-821-7500. They will need the following information to process the credit card: Name, billing address, zip code, credit card #, expiration date, and three-digit security code, and amount of donation.
